Spinal hydatidosis (HS) is an anthropozoonosis due to the development in humans of the larval form of Echinococcus granulosus. The intradural extramedullary spinal location is exceptional with a prevalence in endemic areas around 1 to 4% depending on the series. Manifestations are rare and involve less than 1% of patients with hydatid disease. The diagnosis is epidemiological, clinical, radiological and biological. Surgical removal is the radical treatment sometimes associated with Albendazole 400 mg for several months or years. We report a case of HS in a 10-year-old child who consulted for back and intercostal pain associated with intermittent claudication of the lower limbs. The diagnosis was confirmed by hydatid serology and magnetic resonance imaging showing an extended cystic-like intradural process between D4 and D10, measuring 140 mm in height and 19 mm in thickness, hypo-intense on T1, hyper - intense in T2, homogeneous. The patient was operated on and we carried out drainage, followed by debridement, curettage and posterior decompression with stabilization. The evolution was marked by the achievement of perfect autonomy after 08 months of functional rehabilitation.
